We are currently hiring for the following positions:

Hospice RN Case Manager

Journeys Hospice is hiring. We are accepting applications/resumes for Hospice RN Case Managers. The Hospice Case Manager is a full time position and includes participation in our on-call rotation.

Working as part of an interdisciplinary team, the Case Manager meets the needs of patients facing end of life. He/She completes initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs and develops a plan of care that establishes goals and intended outcomes.

Qualified candidates for this position:
Must possess a nursing degree from an accredited college or university.
Must possess a current, unencumbered, active license to practice as an RN in this state.
Must have a valid driver's license and current automobile insurance.
Must understand philosophy of hospice concept and the needs of the terminally ill.
Must possess the ability to work independently without supervision; must be a self-starter and use good judgment in difficult situations.
Must possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles and ages.
Must be willing to be on call and place client's needs first.

Journeys Hospice is an independent locally owned and operated agency. We offer competitive pay, paid time off, health insurance stipend, on-call pay, cell phone reimbursement, and mileage reimbursement.

Weekend Hospice Nurse

Journeys Hospice is hiring. We are now accepting applications/resumes for a Weekend Hospice Nurse. This is a part time position providing on-call coverage. Pay for this position is negotiable and will be dependent on experience and type of license (RN or LPN).

Qualified candidates for this position:
Must possess a nursing degree from an accredited college or university.
Must possess a current, unencumbered, active license to practice in this state (LPN or RN).
Must have a valid driver's license and current automobile insurance.
Must understand philosophy of hospice concept and the needs of the terminally ill.
Must possess the ability to work independently without supervision; must be a self-starter and use good judgment in difficult situations.
Must possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles and ages.
